Line out (sound card) -> mixer stereo channel in (or 2x mono).
Mixer master out -> monitoring system.

Any mixer channel with direct out/insert/aux send prefader -> sound card analog in.

Disable direct monitoring in your DAW and you're in the game.

You could plug a mixer into (or just skip the mixer altogether) a Presonus Firestation, Firepod (now called FP something) or another one of their products.

The Firestation, and Firepod both offer multiple inputs and outputs (including 8 mic/line inputs), using fire wire to get into and out of your PC.

A Texas Instruments Firewire card for your PC is the way to go, and very inexpensive. The Firestation or Firepod are about $700...
