there are a couple of comercial units out there

you can also find some DIY ideas on my sites

an unused Mic-pre and some resistors could y cord your way into the Headphone Amp.

if you have a spare input just plug in a mic, make a track for that channel, and record enable so they can hear you, yeah you record the talkback but you just get rid of it after.

    American Sign Language?

No, seriously, you probably already have what it takes. A mic in your end, a mic pre and some way of changing the volume. Mix to taste into the headphone output.

It might help us giving suggestions if you tell what equipment you have.

I use a Presonus Central Station for all of my monitor routing, then into a Furman HDS-6 with a few headphone stations in the live room. This unit (HDS-6) is cabled solely by CAT-5 which make is very easy to link up the stations. Works excellent and not too heavy on the wallet.

Don't laugh, I found out about this box at Radio Shack from someone on the "Acoustic Recording" forum (Joe H?). I don't have the skew#, but RS has a "paging microhone" that has a switchable chime and a line level output. Push the button, it sends out the chime and then you can talk via any powered speaker. I got one and plug it into a pair of these little Yamaha powered speakers. Heck, you can run it into ANYTHING, even a geetar amp! I know it didn't cost me even $50.00 and it works like a champ! Check out the Radio Shack website for "paging microphone".
