I use the following Sennheisers & Neumanns:

Sax - 421, mic boom low in, or just outside the bell.
Trombone - 441, straight in.
Trumpet - 409, in front - about 4 inches away.
Flute - Neumann KM84, a few degrees away from the mouth (try to avoid breath noise, get's pretty gnarly sounding if it's too close).
